On the results of the technical investigation of an incident
On July 14, 2024, at the construction site "Apartment building No. 33 on Mira Street in Kalinkovichi", during a storm front, the braces of the sections of the tower crane KBM-401P (manufactured in 2009 by RUP "Mogilev Plant "Strommashina"), owned by OAO "Mozyrpromstroy", were damaged (deformed). Due to adverse weather conditions and the absence of electricity, construction and installation work using the specified crane at the site was not carried out during this period.
During the investigation, it was established that the deformation of the lifting crane's metal structures occurred as a result of wind impact with the slewing mechanism not braked. The causes of the incident were the following violations of industrial safety requirements:
1. The instruction for safe work for the tower crane operator (crane operator) (operating instructions) was developed without taking into account the requirements of the operating manual for the KBM-401P tower crane (the requirement to unbrake the tower crane's slewing mechanism after work cessation is missing);
2. The crane work production project does not provide for the safe operation conditions of several lifting cranes whose operating zones intersect;
3. On the eve of the incident, after the tower crane ceased operation, the crane operator did not unbrake the slewing mechanism.
