In accordance with clause 3 of the "Regulations on the Procedure for Issuing a Unified Ticket, Conditions and Terms of its Use," approved by Resolution of the Council of Ministers of 14.06.2006 No. 748 (hereinafter referred to as the Regulations), a unified ticket is issued within a 5-day period from the date of provision of state support to orphans, children left without parental care, as well as persons from among orphans and children left without parental care.
The issuance of a unified ticket to orphans, children left without parental care, residing in family-type children's homes, guardianship families, or foster families is carried out by the education department (division) of the local executive and administrative body that made the decision to place them in a family-type children's home, guardianship family, or foster family.
In accordance with clause 14 of the Regulations, copies of issued unified tickets, certified by the head of the education department (division) at the child's place of residence (location), are kept in the personal files of orphans and children left without parental care.
In violation of these legislative requirements, unified tickets were not issued to Ambrazheichyk A.A., born on 08.11.2007 (state support provided by the decision of the Lepel Executive Committee of 27.05.2016 No. 518), and to Goryacheva E.A., born on 01.08.2013 (Executive Committee decision of 19.02.2016 No. 167, state support provided from 22.02.2016).
Copies of issued unified tickets are missing from the personal files of Stepanets S.S. (issued on 16.09.2016 according to the unified ticket registration log), Mazgo K.O. (issued on 15.07.2016), and Filipieva M.D. (issued on 19.04.2016).
Identified violations of legislation on the procedure for issuing unified tickets harm the rights and legitimate interests of orphans and children left without parental care.
